Cotinine Nicotine Urine Test

What it measures

Drugs and Psychoactive Substances screenings

Clinical significance

This test measures the amount of cotinine in your urine. Cotinine is the main breakdown product of nicotine, the addictive chemical found in tobacco products and e-cigarettes. Measuring cotinine is more accurate than measuring nicotine itself because nicotine leaves your body within a few hours, while cotinine stays for up to several days.

This test is often used to evaluate nicotine exposure, monitor progress in smoking cessation programs, or adjust the dose of nicotine replacement therapy (such as patches or gum). It can also help determine whether someone has been exposed to secondhand smoke.

🔍 Why this test is important:

Nicotine exposure affects the cardiovascular system, metabolism, and respiratory health. Cotinine testing helps you and your healthcare provider:

  • confirm whether nicotine use has truly stopped,
  • monitor the effectiveness of a quit-smoking program,
  • assess the degree of exposure to tobacco or vaping products,
  • verify abstinence from nicotine before surgery or participation in certain health programs.

🧪 What do my results mean:

  • < 10 ng/mL – typical for nonsmokers or people with no recent nicotine exposure.
  • 10–30 ng/mL – may indicate light smoking or exposure to secondhand smoke.
  • 30–100 ng/mL – suggests occasional or moderate smoking.
  • > 500 ng/mL – typical for regular or heavy smokers.

(Cut-off values may vary slightly between laboratories.)

⏱ How long cotinine stays in your body:

Cotinine can usually be detected in urine for up to 3–4 days after nicotine exposure. In heavy or chronic smokers, it can persist for up to a week.

⚠ Factors that may affect results:

  • Nicotine replacement products (patches, gum, lozenges, or sprays) will raise cotinine levels.
  • Secondhand smoke can cause mild elevation.
  • Individual metabolism (influenced by age, sex, and liver function) may affect how quickly cotinine is cleared.

💡 What you can do:

If you are trying to quit smoking or vaping, cotinine testing can help you track your progress. Combining behavioral support, stress-management techniques, and nicotine replacement therapy (if recommended by your doctor) significantly improves success rates.
