Clinical significance

Vitamin E is a fat-soluble antioxidant, primarily in the form of alpha-tocopherol in the blood. It plays a key role in protecting cell membranes from oxidative stress, supports red blood cell production, maintains immune and nervous system health, and has anticoagulant properties.

This test measures serum alpha-tocopherol and can be done independently or as part of a fat-soluble vitamin panel.

When is it ordered?

  • Neurological symptoms of unknown origin (e.g. muscle weakness, ataxia, visual disturbances)
  • Memory problems or cognitive decline
  • Suspected fat malabsorption (celiac disease, cystic fibrosis, liver disease, chronic pancreatitis)
  • Patients receiving parenteral nutrition or fat-blocking drugs
  • Antioxidant status evaluation in chronic/inflammatory diseases
  • High-dose vitamin E supplementation monitoring
  • Possibly: depression, anxiety, infertility, Alzheimer’s

Symptoms of Vitamin E Deficiency:

  • Ataxia, peripheral neuropathy, muscle weakness
  • Hemolytic anemia
  • Retinopathy
  • Cognitive issues and fatigue
  • Increased infection susceptibility

Test Accuracy:

Serum vitamin E levels are reliable but should be interpreted with lipid levels. Alpha-tocopherol is transported via lipoproteins. Ideally, use the alpha-tocopherol to cholesterol ratio.

Reference ranges (μmol/L):

  • Men: 7.8–33.2
  • Women: 10.2–43.6
  • Pregnancy (2nd–3rd trimester): 15.6–55.7

Vitamin E Excess:

  • May increase bleeding risk (vitamin K interference)
  • GI symptoms, headaches, fatigue
  • Caution with anticoagulants
  • UL: 300 mg/day (approx. 450 IU)

  • Nuts and seeds
  • Green leafy vegetables (spinach, broccoli)
  • Whole grains
