Alkaline phosphatase (ALP) is an enzyme found primarily in the liver, bile ducts, and bones. The ALP blood test measures the level of this enzyme to help assess liver and bone health.

ALP is often tested as part of a liver panel or together with other markers such as gamma-glutamyl transferase (GGT), bilirubin, ALT, and AST to better understand the source of changes.

ALP plays a role in processes related to liver function and bone metabolism. Changes in ALP levels may help differentiate between liver-related and bone-related conditions when interpreted together with other laboratory markers.

↑ What does a high ALP level mean?

Elevated ALP levels may be associated with:

  • Liver conditions (e.g., bile duct obstruction, liver inflammation)
  • Increased bone activity (e.g., bone growth, bone disorders)

If other liver markers (ALT, AST, bilirubin, GGT) are also elevated, the source is more likely liver-related. If these markers are normal, the elevation may be associated with bone metabolism.

In some cases, additional testing (such as ALP isoenzymes) may be used to determine the source of elevation.

Low ALP levels may be associated with:

  • Nutritional deficiencies (e.g., zinc deficiency, malnutrition)
  • Certain metabolic or genetic conditions (e.g., hypophosphatasia)
