Clinical significance

The ENA test is a blood test that checks for antibodies in your blood. These antibodies are proteins that can be extracted from the cells in your body, which is why the test is called "Extractable Nuclear Antigens" (ENA).

These antibodies are types of autoantibodies. Autoantibodies are made by the immune system to help fight off infections like bacteria and viruses. However, autoantibodies mistakenly target and attack the body’s own healthy tissues.

The ENA test is sometimes called an ENA panel because it consists of 4 to 6 blood tests taken from a single blood sample. Each ENA panel checks for different autoantibodies.

The ENA test is one step that your healthcare team may use to help diagnose diseases where the immune system attacks healthy tissues, known as autoimmune diseases.

🔍 Interpretation of Results:

Positive Result:

A positive result from the ENA test is commonly seen in patients with the following autoimmune diseases:

  • Systemic Lupus Erythematosus (SLE)
  • Skin forms of lupus
  • Systemic scleroderma (both diffuse and limited forms)
  • Sjögren’s syndrome
  • Mixed connective tissue disease
  • Dermatomyositis

In rare cases, positive results may also be associated with viral infections and chronic inflammatory diseases.

Negative Result:

  • A negative result lowers the likelihood of systemic lupus erythematosus and other systemic diseases in the patient. No antibodies against extractable nuclear antigens are found in most patients with autoimmune liver diseases, rheumatoid arthritis, and juvenile rheumatoid arthritis.

Doubtful Result:

  • A doubtful result may occur if the antibody levels are low (near threshold) or due to individual non-specific serum interferences. In such cases, it may be recommended to repeat the test at the discretion of the treating physician.
